GCHIP meets on a monthly basis and consists of three subcommittees that address the health priorities identified through the Community Health Needs Assessment (CHNA). GCHIP leads a comprehensive regional triennial CHNA effort, to identify and prioritize the top health needs in the region and creates Community Health Improvement Plans (CHIP) to address those health needs. As part of the CHNA process, GCHIP members play a vital role and provide key input.
The current 2019-2022 health priority areas identified are promoting healthy lifestyles, promoting behavioral health and promoting access to care. GCHIP guides the work of three subcommittees that address each of the priority areas. The three subcommittees consist of representatives from partner organizations who are interested in and committed to improving the health of the community. GCHIP member collaborate to ensure their specific CHIP goals are met and progress is tracked.
